Care coordination takes a central place in the current transformation of the healthcare system. Today, health systems, insurers, and hospitals are all stressing the importance of coordination as a vital instrument in enhancing patient care and improving health outcomes. It is critical in improving patient health and satisfaction but is also essential in reducing the cost of healthcare. Over the years, care coordination has been a vital role of registered nurses. Aside from being a competency, it is a core professional standard for the RNs as they develop care plans that revolve around the preferences and needs of patients. ANA Position on Care coordination

The American Nursing Association (ANA) (n.d) provides general guidelines about the role of registered nurses in care coordination. This way, it will be possible to tailor care to a patient across the coordination team. Sharing knowledge with team members also promotes collaboration to ensure the development of a proactive care plan to improve a patient’s health. The ANA also reiterates that RNs should help patients manage their healthcare goals in myriad ways, including coordinating community resources to help patients achieve health goals. According to AHRQ, care coordination is about deliberately coordinating patient care activities by sharing crucial information across the healthcare team to achieve better healthcare outcomes for patients.

Both ANA and AHRQ reiterate linking patients to community resources, communicating and sharing knowledge and information, assessing patient needs and goals, and helping patients self-manage their healthcare goals.

Contrast

AHQR highlights a cultural shift in interpersonal and relational practice at all levels, while ANA cites limited engagement with community partners as hindering coordination care.

QSEN Competency

This section addresses Quality and Safety in Educating Nurses, popularly known as QSEN competencies. QSEN (n.d) competencies refer to the competencies established for nursing to make them proficient in service. They comprise six elements: quality improvement, informatics, safety, evidence-based practice, teamwork and collaboration, and patient-centered care. These elements were established to aid in the continuous improvement of nursing nurses and improve patient outcomes in healthcare settings (QSEN, n.d). Even though most of these competencies are already being practiced in the healthcare field, there is a need for policymakers and experts to do more to integrate the remaining competencies into everyday practice.

Patient-centered care is the first competency, and it involves including patients in significant decisions to aid healthcare delivery that serves the unique needs of every patient.

Teamwork and collaboration come second in the list and are about a collaboration among different healthcare professionals to share decisions and ensure excellent healthcare outcomes for patients.

The third is evidence-based practice (EBP). It involves the utilization of present evidence in making decisions while attending to the healthcare needs of patients.

Fourthly is quality improvement (QI). It is about collecting and evaluating data to improve the outcome for patients.

Safety is the fifth competency, and it is about protecting patients from whatever kind of harm.

Informatics is the final competency. It involves utilizing technology to improve the quality of patients’ healthcare services.

Analysis of the ANA Position on the Nursing Profession and Public

This section discusses the ANA’s position on nursing practice and public policy. The ANA develops this position every once to reflect the changing healthcare practice and ecosystem. The position statement is crucial in healthcare practice because it educates consumers and enhances and guides the nursing profession.
